First of all, this is not the “big question”. The big question, and the one Nolan actually intended audiences to question, is whether or not the ending was a dream, not whether the whole movie was a dream. Obviously when you consider all things, the ending is without a doubt reality. But I suppose it does offer some ambiguity for the one timer viewer.

From what the movie gives, I assume Miles was Cobb’s professor at college and may have met Mal when Miles thought him about the dream machine.
